The -4 revision is pre-MIM and pre-lock, and the frame is drilled and tapped under the rear sight. It features adjustable sights, a serrated top rib and topstrap, and ships with its factory box and S&W revolver manual.

The 686-4 PowerPort pairs the strength of the L-frame with a stainless steel construction across the frame, cylinder, and 6 inch barrel. The factory Power Port at the muzzle and the full underlug barrel are hallmark features of this variant, complemented by a serrated top rib and topstrap.
It is a double-action/single-action revolver with a six-shot fluted cylinder. Sight regulation is handled by a fully adjustable rear sight and a black ramped front sight, and it wears black rubber finger-groove grips.
As a -4 revision, it is pre-MIM and pre-lock. The frame is drilled and tapped beneath the rear sight for optics mounts. The yoke cut is marked "MOD. 686-4."
Factory rollmarks include "SMITH & WESSON" on the barrel left, "S.&W. .357 MAGNUM" on the barrel right, and the standard "MADE IN U.S.A. / MARCAS REGISTRADAS / SMITH & WESSON / SPRINGFIELD, MA." The revolver is United States made and aligns with 1990s production features.
