The Model 19-4 engineering change moved the cylinder gas ring from the yoke to the cylinder, a running update within the Combat Magnum line. Built on the K-frame with a square-butt grip, it retains the classic six-shot cylinder and a smooth double-action/single-action trigger system.

The 6-inch barrel adds a full-length rib and an ejector-rod shroud, pairing with an adjustable micrometer rear sight and a ramped, serrated front sight for a clear sight picture. Its blued carbon-steel construction presents the traditional S&W finish of the period.

Chambered in .357 Magnum, it also accepts .38 Special, offering flexibility for training and duty-style loads. The Model 19 originated as the .357 Combat Magnum developed with Bill Jordan and saw wide police adoption in the 1960s–1970s, underscoring its historical relevance.

Production for the 19-4 variant ran circa 1977–1982, with the observed serial range indicating a late-1970s timeframe. Checkered Goncalo Alves target stocks with inset S&W medallions complete the period-correct configuration, and the revolver is made in the USA.
